Ethical institutions in the health sector include various entities such as the Indonesian Medical Council, the Dental Council, the Indonesian Health Workers Council, and the Indonesian Medical Discipline Honorary Council (MKDKI). These institutions act as regulators, supervisors, and policymakers related to codes of ethics and standards for health professions. Additionally, there are also the Medical Ethics Honorary Council (MKEK), the Hospital Ethics and Law Committee (KEHRS), and the Provincial Hospital Supervisory Board (BPRS), which have specific roles in enforcing ethical standards. However, overlapping authorities can slow down the enforcement of ethics. The utilitarian perspective evaluates actions based on the overall benefits and harms to society, although it faces challenges in objectively measuring pleasure and pain. This study aims to analyze the interrelation between ethical institutions in enforcing professional ethics, the authority of ethical enforcement in hospital health services, and the utilitarian perspective on the authority of these ethical institutions. The method used is normative legal research with descriptive qualitative analysis. The results show that MKEK and MKDKI play important roles in maintaining medical professional standards in Indonesia, while various committees in hospitals also play roles in enforcing ethics and the quality of health services. From a utilitarian perspective, effective ethical enforcement can improve general welfare. However, the dualism of ethical institutions and communication challenges between doctors and patients require more attention to reduce potential medical disputes. In conclusion, the synergy between various ethical institutions and the improvement of communication and understanding of professional ethics can support the creation of better and more ethical health services.
