Manual recording of student violation data often leads to difficulties in data retrieval, monitoring, and evaluation processes by the Counseling Guidance Office. Therefore, this study aims to design and develop a web-based Student Violation Data Recording System for the Counseling Guidance Office at Madrasah Tsanawiyah Al Washliyah Medan Krio. The system development follows the Waterfall model, consisting of five main phases: requirements analysis, system design, implementation, testing, and maintenance. The system is designed to help counseling teachers easily record, access, and follow up on student violations digitally, quickly, and accurately. The design process includes developing an intuitive user interface and an efficient database structure to support effective information management. Testing results show that the system functions properly according to the defined functional and non-functional requirements. The system also provides real benefits by improving work efficiency and accuracy in student violation data management. With this system, the data recording and evaluation process in the Counseling Guidance environment becomes more professional and digitally organized. This research is expected to offer an information technology solution that can also be applied in other educational institutions to support optimal data management.
